Office Policies
  • Insurance: We accept most health insurance plans, including BCBS, Aetna, United Healthcare, Cigna Healthcare, Private Healthcare Systems, Magellan Behavioral Health and many others. When setting up an initial appointment, we will collect insurance information in order to verify your benefits prior to your visit and answer any questions you may have.
  • Special Programs: For additional information on Suboxone (buprenorphine) treatment and hypnosis, please call the office and ask for our on-duty Suboxone intake coordinator.
  • Scheduling Appointments: Please call the office and speak to one of our friendly staff members to set up an appointment.
  • What To Bring: Below, you will find forms for you to complete and bring with you to your appointment, which will provide us with the necessary health information including the names and addresses of therapists, physicians, health history, and medications. You may also choose to bring any current medications to your first appointment. Please bring your health insurance card(s) and a photo ID for security purposes.
