Appointment
We are currently
accepting new patients. To
schedule your initial
appointment, please call our
office at 404.685.9414 and
speak to one of our staff
members. We schedule
appointments Monday thru
Friday from 9:15am to 5:15 pm.
The initial visit will last
about 30 minutes. For

Special
Programs
Dr. Grumet sees
patients for Suboxone
(buprenorphine) treatment.
For more information on these
treatments, please call the
office and ask for our on-duty
Suboxone intake coordinator.
Urgent, same day, after hours
appointments may be available,
but are not covered by
insurance.
Insurance
We accept most health
insurance plans, including
Aetna, United Healthcare,
Cigna Healthcare, Private
Healthcare Systems (PHCS), and
several others. We
also accept self pay
patients. When
setting up an initial
appointment, we will collect
insurance information in order
to verify your benefits prior
to your visit and answer any
questions you may have.
What to Bring
Below, you will find forms for
you to complete and bring with
you to your appointment, which
will provide us with the
necessary health information
including the names and
addresses of therapists,
physicians, health history,
and medications. You may also
choose to bring any current
medications to your first
appointment. Please bring your
health insurance card(s) and a
photo ID for security
purposes.

forms in advance will save you
time on your initial visit. If
you have difficulties

the forms, we will provide
them to you at the office. If
you have to fill them out in
the office, please arrive to
