    Cancellation

    APPOINTMENT CANCELLATION POLICY AND CANCELLATION FEES

    • You MUST cancel at least 24 hours in advance of your appointment to avoid a fee.
    • There is a $105.00 cancellation fee with less than 24 hours notice.
    • If you are a no-show for your appointment, there is a $105.00 missed appointment fee.
    • Monday appointments (or Tuesday appointments when Monday is a holiday) cancellations must be made before 3pm on the previous Friday to avoid a cancellation fee.
    • Early cancellations give us time to schedule someone who has been on the waiting list for a needed appointment.

    You will receive an email acknowledging your cancellation. It is important you keep this email as it is your receipt you canceled 24 hours before your appointment.  If you do not receive an email within 30 minutes of completing this form, then we did not receive your cancellation (make sure to check your spam folder).  You need to call the office to cancel.
