Do you accept insurance?
All patients are “self-pay”. We are unable to accept insurance plans.
What are your fees?
Current fees for services:
- New Patients: $350
- Follow-up: $150
- Emergency/Walk-in – $250
- Prior Authorization – $35
- Cancellation/No Show – $95
(24 hour cancellation required. Monday appointments must be cancelled before 3:30pm on Friday)
What is a prior authorization for prescriptions?
Prior Authorization means the same as Pre-authorization and abbreviated as PA. If your insurance company requires a medication prior authorization, there is a $35 charge.
How do I MAKE an appointment?
How do I CANCEL an appointment?
If you must cancel your appointment, we ask that you provide us with as much notice as possible, but a minimum of at least 24 hours is required. If you cancel your appointment with less than 24 hours notice, there is a $95 fee. Monday appointment cancellations must be received before 3:30 pm on the previous Friday to avoid a fee.
